Job SummaryThe post holder is a Clinical Pharmacist who works within their professional boundaries to support and collaborate with a team of pharmacists and support staff in general practice. The pharmacist will be supported by a senior clinical pharmacist who will develop, manage and mentor them.
- Work as part of a multi‑disciplinary team in a multi‑faceted role with patient‑facing and organisational responsibilities.
- Aid PCN member practices in chronic disease management and conduct clinical medication reviews to manage patients with polypharmacy.
- Provide primary support to general practices regarding prescription and medication queries, including repeat prescriptions, acute requests and medicines reconciliation on transfer of care.
- Deliver clinical leadership on medicines optimisation, quality improvement and manage aspects of the Quality and Outcomes Framework and enhanced services.
- Assist the PCN in integrating with community and hospital pharmacy to improve patient outcomes, access to healthcare and workload management.
- Develop towards a non‑medical prescriber role and participate in the CPPE Pharmacists in General Practice education course.
- Masters degree in Pharmacy (mandatory).
- Registration with the General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society is desirable.
- Independent Prescriber qualification is desirable.
- Minimum of 2 years post‑qualification experience as a pharmacist.
- Appreciation of general practice prescribing and rational prescribing strategies.
- Excellent interpersonal, influencing, negotiating, written and verbal communication skills.
- Ability to plan, manage, monitor, advise and review general medicine optimisation issues.
- Good IT skills and ability to analyse complex technical information.
- Recognise priorities, problem‑solve and refer to seniors or GPs when appropriate.
- Work under pressure and meet deadlines; influence stakeholders to comply with recommendations.
- Work effectively independently and as part of a team, demonstrating accountability for professional expertise.
- Self‑motivation and adaptability.
- Full driving licence.
- Current CRB.
- Safeguarding adults and children Level 3.
- Information Governance toolkit completion.
- Immunisation status.
- Basic Life Support training.
