Job Summary

The post holder is a Clinical Pharmacist who works within their professional boundaries to support and collaborate with a team of pharmacists and support staff in general practice. The pharmacist will be supported by a senior clinical pharmacist who will develop, manage and mentor them.

  • Work as part of a multi‑disciplinary team in a multi‑faceted role with patient‑facing and organisational responsibilities.
  • Aid PCN member practices in chronic disease management and conduct clinical medication reviews to manage patients with polypharmacy.
  • Provide primary support to general practices regarding prescription and medication queries, including repeat prescriptions, acute requests and medicines reconciliation on transfer of care.
  • Deliver clinical leadership on medicines optimisation, quality improvement and manage aspects of the Quality and Outcomes Framework and enhanced services.
  • Assist the PCN in integrating with community and hospital pharmacy to improve patient outcomes, access to healthcare and workload management.
  • Develop towards a non‑medical prescriber role and participate in the CPPE Pharmacists in General Practice education course.
Qualifications
  • Masters degree in Pharmacy (mandatory).
  • Registration with the General Pharmaceutical Council.
  • Membership of the Royal Pharmaceutical Society is desirable.
  • Independent Prescriber qualification is desirable.
Experience
  • Minimum of 2 years post‑qualification experience as a pharmacist.
  • Appreciation of general practice prescribing and rational prescribing strategies.
  • Excellent interpersonal, influencing, negotiating, written and verbal communication skills.
  • Ability to plan, manage, monitor, advise and review general medicine optimisation issues.
  • Good IT skills and ability to analyse complex technical information.
  • Recognise priorities, problem‑solve and refer to seniors or GPs when appropriate.
  • Work under pressure and meet deadlines; influence stakeholders to comply with recommendations.
  • Work effectively independently and as part of a team, demonstrating accountability for professional expertise.
Other Essential
  • Self‑motivation and adaptability.
  • Full driving licence.
  • Current CRB.
  • Safeguarding adults and children Level 3.
  • Information Governance toolkit completion.
  • Immunisation status.
  • Basic Life Support training.
