Overview

As part of a multidisciplinary team the post holder will work autonomously to provide a high-quality service to patients attending urgent care services. This includes all ambulatory areas which see / treat patients presenting with Minor Injury and Minor Illness. Working as an emergency practitioner with a high degree of autonomy, the post holder will manage undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within their own sphere of competence.

Practitioners will aim to enable individuals to maintain or regain independence or ensure a plan is in place for them to prevent unnecessary hospital admission. The post holder will be responsible for ensuring that patients in the Minor Injury and Minor Illness Unit are seen and assessed with treatment plan implemented within the agreed timescales and appropriately processed through the correct Emergency Department (ED) pathway ensuring local referral and escalation pathways are adhered to.

Responsibilities
  • Provide highly specialist care and advise to patients presenting with minor injury or minor illness in unscheduled care settings from assessment of presenting condition through to diagnosis, initial treatment, monitoring of care and treatment and discharge including the administration of medication within agreed clinical guidelines, Non-Medical Prescribing practice patient group directives (PGD's) and protocols.
  • Works to promote and safeguard the interests and wellbeing of patients and colleagues recognises and responds when an adult / child might be at risk from abuse understanding of limits and escalating and referring to others, when necessary, i.e.:
    Safeguarding services.
  • Always acts as a patient advocate to ensure patient focused approach to care and maintaining patient dignity and inclusiveness.
  • Working closely with all staff members to ensure prompt and effective patient assessment and treatment is undertaken, and decisions are swiftly acted on.
  • Actively encourage and enable multi-disciplinary approached to care.
  • The post requires the ability to work under pressure and to deadlines / targets ensuring patient care is delivered in timely manner and to national targets.
