Digital & Transformation Lead - SES PCN

The core objectives of the Digital Transformation Lead is to lead, develop & implement strategic plans for South East Shropshire PCN and their member practices. The post holder will work with clinical and operational teams within the Primary Care Network (PCN) to create a vision that guides service improvements, identifies opportunities for tangible positive transformation, and provides the inspiration, resources, and tools to deliver changes collaboratively.

The post holder will support the development of this vision in alignment with the Integrated Care System (ICS) strategy. The post holder will plan, coordinate and directly support the delivery of transformation activity in general practice within a PCN, with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services.

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are currently a GP partnership of 29 practices with 38 surgeries, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.
