We are seeking a skilled and motivated Community Psychiatric Nurse (CPN) to join the CMHT Forensic Interface Provision, a specialist function within Community Mental Health Services. The post holder will be based within and work closely alongside Small Heath CMHT.
The provision delivers enhanced, time‑limited and high‑intensity community interventions to people with complex mental health needs at critical points in their recovery. This includes individuals stepping down from secure and forensic services, as well as CMHT service users requiring additional therapeutic input, coordinated risk management and continuity of care to remain safely supported in the community.
The role offers an opportunity to strengthen pathways between secure care and community services, improve transitions, and support recovery, stability and independence. You will work as part of a small, specialist MDT alongside partners across health, social care and the criminal justice system.
To support the complexity of the work, the role operates with a reduced caseload, typically around 15 service users, subject to clinical need and service pressures.
Responsibilities- Act as Care Coordinator for a highly complex caseload, delivering time‑limited, high‑intensity community mental health interventions to adults with complex and high‑risk needs.
- Lead assessment, care planning and risk management under the Care Programme Approach (CPA).
- Support safe transitions from secure and forensic services.
- Undertake statutory Social Supervision (with training provided).
- Work closely with CMHT colleagues, secure services, criminal justice partners and social care to promote recovery, stability and continuity of care.
- Full support and provision of Social Supervision training.
- Part of an 18‑month pilot to evidence and embed a sustainable model of care.
- If further funding is not secured, the post holder would be supported to transition into an alternative CPN role within the service.
