Job Summary
The post holder will work as part of a dynamic team, including commissioners and providers in delivering an effective service supporting people with Learning Disabilities in their CHC journey. The role involves assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care (2009; revised 2018 and 2022), planning and commissioning packages of care (including Personal Health Budgets), supporting individuals, their families and health professionals through the Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility/packages of care.
Mainduties of the job
- Undertake comprehensive professional Continuing Healthcare and Funded Nursing Care assessments for adults over 18 with complex health needs, including assessment, review and monitoring of care funded via NHS Continuing Healthcare within dedicated time scales. This includes retrospective and appeal cases.
- Act as a Case Manager for individual clients within Birmingham Solihull ICB who are receiving continuing health care funding, to assess needs, plan and evaluate service provision to achieve optimal function and appropriate care in the most suitable environment.
- Provide robust, transparent, multi‑agency assessments for NHS Continuing Healthcare and NHS-funded Nursing Care.
- Support the implementation of the National Framework for Continuing Healthcare, including contributing to training programmes and delivery plans to facilitate learning.
- Work with Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care applications; provide professional nursing advice and collaborate with partners to meet patients’ health care needs.
- Assist in observing quality of care in care homes, identify unmet needs and promote improvements in care standards.
- Identify opportunities to improve value for money and ensure care packages meet clinical needs within available funding.
- Prepare Continuing Health Care Assessments for the Continuing Care eligibility Panel and participate in appeals, reviews and panels as required.
- Provide information resources and guidance to the public, professionals and external agencies; maintain knowledge of the National Framework for Continuing Healthcare and Funded Nursing Care.

- Work with health, social care and independent sector colleagues to ensure care management processes are appropriate; may present assessments to the BSOL continuing care panels for ratification.
- Follow the National Framework for NHS Continuing Healthcare; organize, co‑ordinate and participate in MDT eligibility recommendations verified by ICB decision makers.
- Manage complex clinical caseload including assessments, reviews (3/12 CHC reviews, unscheduled reviews, Fast Tracks) and implementation of Personal Health Budgets; ongoing case management of CHC, Mental Health and Learning Disabilities.
- Ensure assessments are robust, transparent and multi‑agency where required; support seamless implementation of the National Framework and contribute to training and learning.
- Provide advice, screening and coordination of assessments against the National Framework within nursing homes; support Nurses in processing Funded Nursing Care and Continuing NHS Health Care applications.
