We are seeking an experienced and highly skilled Advanced Clinical Practitioner (ACP) to join our multidisciplinary team.
This is a senior clinical role for a practitioner who is able to work autonomously, using advanced clinical knowledge and judgement to assess, diagnose and treat patients.
The ACP will manage patients with both acute and long-term conditions, helping to reduce pressure on GP services and ensuring patients receive timely, effective care.

The role also includes undertaking home visits where clinically appropriate and supporting the supervision of students where appropriate.
Main duties of the jobAs an Advanced Clinical Practitioner, you will work autonomously to assess, diagnose and treat patients with a wide range of acute and long-term conditions.
You will run your own clinics, carry out home visits when required, and support timely access to care by managing patients who would otherwise need a GP appointment.
You will play a key role in improving patient outcomes through high-quality clinical decision-making, health promotion and preventative care. Alongside clinical work, you will support service development, contribute to quality improvement, and provide supervision to students and developing staff where appropriate, helping to build a strong and skilled clinical team.

Job responsibilitiesIndependently assess, diagnose and treat patients within scope of practice
Manage patients presenting with acute and chronic conditions
Undertake home visits where clinically appropriate
Prescribe as an independent prescriber and undertake medication reviews
Request, interpret and act upon investigation and pathology results
Deliver chronic disease management and contribute to QOF and enhanced services
Promote health, wellbeing and disease prevention
Ensure safeguarding responsibilities for adults and children are met
Maintain accurate and timely clinical records using SNOMED coding
Work collaboratively with the multidisciplinary team
Contribute to clinical governance, IPC and medicines management
Support, supervise and mentor nursing and clinical staff
Supervise students and learners where appropriate, supporting their development
Develop and lead on an area of specialist clinical interest
Support service development in line with national and local priorities
Person Specification Qualifications- Registered healthcare professional with a recognised professional body
- Educated to Masters level in Advanced Clinical Practice in line with the Multi-professional Framework for Advanced Clinical Practice in England
- Evidence of working at an advanced level of practice
- Qualified independent prescriber
- Evidence of relevant professional and academic qualifications
- Eligible for professional revalidation
- Advanced Life Support (ALS) and/or Paediatric Life Support (PALS)
- Evidence of specialist clinical training or extended skills relevant to primary care
- Excellent communication skills with patients, carers and colleagues
- Strong clinical reasoning and decision-making skills
- Ability to work autonomously and manage complex, unpredictable situations
- Effective time management and organisational skills
- Commitment to equality, diversity and inclusion
- Understanding of safeguarding adults and children
- Knowledge of clinical governance and quality standards
- Ability to use IT systems for clinical recording and communication
- Professional, ethical and accountable approach to practice
- Experience in service development or pathway redesign
- Ability to mentor, teach or supervise students and staff
- Leadership experience within a clinical team
- Knowledge of QOF, enhanced services and primary care contracts
- Flexibility to support service needs, including occasional work outside core hours
