Band 6 Community Psychiatric Nurse - CMHT Forensic Interface Provision

We are seeking a skilled and motivated Community Psychiatric Nurse (CPN) to join the CMHT Forensic Interface Provision, a specialist function within Community Mental Health Services. The post holder will be based within and work closely alongside Small Heath CMHT. The provision delivers enhanced, time-limited and high-intensity community interventions to people with complex mental health needs at critical points in their recovery.

This includes individuals stepping down from secure and forensic services, as well as CMHT service users requiring additional therapeutic input, coordinated risk management and continuity of care to remain safely supported in the community. The role offers an opportunity to strengthen pathways between secure care and community services, improve transitions, and support recovery, stability and independence. You will work as part of a small, specialist MDT alongside partners across health, social care and the criminal justice system.

To support the complexity of the work, the role operates with a reduced caseload, typically around 15 service users, subject to clinical need and service pressures. The post holder will undertake Social Supervision training, which will be fully supported and provided. This post forms part of an 18-month pilot to evidence and embed a sustainable model of care. Should further funding not be secured, the post holder would be supported to transition into an alternative CPN role within the service.

This is a substantive contract.

Main duties of the job

The post holder will act as Care Coordinator for a small, highly complex caseload, delivering time-limited and high-intensity community mental health interventions to adults with complex and high-risk needs. The role involves leading assessment, care planning and risk management under the Care Programme Approach (CPA), supporting safe transitions from secure and forensic services, undertaking statutory Social Supervision (with training provided), and working closely with CMHT colleagues, secure services, criminal justice partners, and social care to promote recovery, stability and continuity of care.

Person Specification Qualification RMN
  • Significant post registration experience in Mental Health.
  • Ability to demonstrate skills relating to engaging Service User Group.
  • NMC registration
  • Risk assessment skills specific to complex, high risk Service Users.
  • Degree/Diploma
  • Evidence of experience as a community psychiatric nurse
  • Demonstrates knowledge of the issues surrounding community mental health services.
