This is a full-time, on-site role located in Birmingham for a Discharge Pathway Officer based in University Hospitals Birmingham. The officer will oversee case management, providing personalized support plans to help individuals transition from homelessness to stable housing. Responsibilities include coordinating with external agencies, assessing client needs, connecting individuals with appropriate services, and monitoring progress. The role also involves maintaining accurate records and fostering partnerships to advance the organization's mission.

• Strong communication, interpersonal, and collaborative skills
• Experience in case management, client needs assessment, and creating support plans
• Knowledge of housing policies, homelessness support services, and resources
• Organizational skills and attention to detail in record-keeping and reporting
• Ability to work independently, manage multiple priorities, and address challenges effectively
• Understanding of safeguarding policies and commitment to supporting vulnerable populations
• Proficiency in basic computer applications and data management systems
• Relevant experience working in support roles or roles related to housing services is an asset
