Overview
Do you have an interest in Psychosis and want to make a real difference in someone s recovery? The Early Intervention in Psychosis Service (EIP) supports service users aged 16-35 experiencing first episode psychosis (FEP) for up to three years. We work with an assertive outreach model with a flexible, creative and proactive approach to engaging service users and promoting recovery.
The core features of the EIP model are based on NICE guidance for psychosis and schizophrenia (2014) and the new national standards for EIP (2016). EIP has a strong culture of multidisciplinary team working within a supportive environment. You will have ongoing supervision, in-house development and training, as well as annual appraisal and progression opportunities. The role of care coordinator includes delivery of EIP-specific interventions such as Behavioural Family Therapy (full training provided) and Carers support.
We are currently looking for MH nurses, OTs and SWs with professional bodies (e.g., NMC/HCPC) who would like to join one of our EIP teams as a care coordinator.
Responsibilities- The post holder will be a member of the specialist community Early Intervention in Psychosis Service and will hold responsibility for the care coordination of a defined caseload alongside the delivery of NICE-guided clinical interventions.
- Liaise with other local agencies including inpatient and specialist services, social care, as well as Core community and Urgent care teams.
- Behave consistently with the values and beliefs of the organisation and promote these on a day-to-day basis.
- Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism.
- Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.
- Provide a high-quality, community-based service to the children, young people and families within a multi-disciplinary team providing a community-based children and young people mental health service.
