As a Clinical Case Manager for Children in Care within the West Midlands CAMHS Provider Collaborative (WMCPC), you will be at the forefront of a system evolving to meet the urgent needs of children who live in residential care homes.

Working within Toucan’s Integrated Complex Care / Children in Care (CiC) team, you will be part of a multi‑agency collaborative redefining how health and social care support the region’s vulnerable young people.

Working in a small, specialist team driving meaningful change. You will be empowered to innovate, influence, and deliver work that improves lives of children, young peoples, and their families for the future.

We are looking to recruit three skilled professionals (RMN, RNLD or Social Worker) who are ready to advocate and bring fresh thinking to integrated care.

Main duties of the job
  • Provide expert clinical advice and case management for children and young people in residential care homes commissioned by West Midlands Local Authorities.
  • Manage complex, cases with autonomy and professionalism, offering guidance and support to the wider network around the child.
  • Champion needs of Children and Young People in residential placements across the West Midlands.
  • Work collaboratively with Local Authority social care teams, to ensure therapeutic needs are identified and met.
  • Lead proactive case discussions, triage referrals, and unlock access to innovative services like Integrated Residential Outreach Care (IROC).
  • Support the Regional Hub and Local Authorities to influence quality and performance standards for residential care providers, ensuring every placement is safe, supportive, and clinically sound.
  • Navigate high‑pressure situations with confidence, compassion, and clinical expertise — making decisions that change lives.
  • Travel across the West Midlands region is required as part of this role.
