Position: Band 6 CPN - Mental Health Outreach Team for Rough Sleepers

To work in the mental health outreach team for homeless rough sleepers located in the city centre but meeting the needs of homeless people city wide. To be responsible for the engagement with and assessment of mental health needs of service users who are identified as rough sleeping in the city via the Rough Sleeping Task Group To refer service users to BSMHFT secondary care services if required and liaise with those services to facilitate care for the service user To signpost service users to other services for homeless people to include primary care, accommodation and substance use services To support a small caseload of homeless rough sleepers while they are working to engage with other commissioned services to enable their movement from rough sleeping to settled accommodation and further recovery To support the delivery of short term therapeutic interventions such as CBT and DBT for service users with common mental health problems To provide professional advice and support to other agencies and other members of the multidisciplinary team.

To participate in multidisciplinary discussions and reviews about service users., The post holder:

  • Will act as an independent practitioner within the community setting, while liaising with other professionals as necessary
  • Will manage a defined caseload and provide supervision to others if appropriate.
  • Will be responsible for the assessment, planning, implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
  • Will be responsible for ensuring effective communication pathways exist.
  • Will be responsible for ensuring the promotion of equality, diversity and rights in accordance with good practice and legislation.
  • Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
  • Will be responsible for actively prescribing within a defined area and for a defined group of service users
