The post-holder will provide a comprehensive Administrative Service for the Complex Discharge Team based within our Complex Discharge Hubs. All activities will underpin the safe and timely discharge of patients and work to reduce patient length of stay. The role involves cross site working on a 7 day rota to include Bank Holidays. This is a 6-month Secondment/Fixed Term Position.
Main duties of the job- Receive and process all referrals triggered by ward staff, record and monitor on the Discharge Hub Management System (DHMS) and other appropriate systems to produce an auditable referral trail and reports at request.
- Follow up on outstanding information by liaising with wards and Complex Discharge Hub colleagues, discussing updates with Complex Discharge Nurses and adding to relevant systems in a clear concise manner as part of a clinical record and for analysis/reports.
- Develop knowledge of the Discharge Hub Management System and use it daily to support discharge planning for patients who have care needs following their acute stay.
- Provide an administrative and secretarial service to the Complex Discharge Hub Team, maintain a diary for the team, provide reports, take minutes as required.
- Order stationery/equipment using the appropriate system, raise requisitions and receipt invoices where applicable; accurate record keeping is essential.
- Receive all incoming calls, prioritise them, log patient referrals when appropriate.
- Participate in the receipt and distribution of all internal and external mail, logging and/or redirecting as appropriate.
- NVQ Level 3 in Business Administration or equivalent qualification and/or experience in an administrative environment.
- RSA level II or equivalent experience.
- Experience of working in an administrative environment, ideally in a Healthcare setting.
- Experience of working with the public/customer service.
- Experience of Microsoft packages including Word, Excel, Outlook.
- Experience of minute taking, preparing and distributing agendas for meetings.
- Experience of working in a busy environment.
- Excellent communication skills – face to face/telephone/email demonstrating empathy and compassion where required.
- Good Interpersonal skills, able to deal with enquiries from staff, patients and visitors professionally.
- Able to produce correspondence with minimal assistance.
- Accuracy and attention to detail.
- Able to prepare and research basic information.
- Willing to undertake routine/repetitive work.
- Able to prioritise work/multitask and meet deadlines.

- Ability to work weekends on a rota basis which includes weekends and bank holidays.

- Knowledge of Trust IT Systems.
- Knowledge of hospital discharge pathways.
