Job Description
To manage and standardize the amendment and correction process across the enterprise. To ensure timely, accurate, and compliant execution of amendment requests submitted by patients or their authorized representatives, in accordance with HIPAA and applicable state regulations. To collaborate with compliance, risk management, health information management (HIM), clinical teams, compliance, and IT to maintain consistent workflows, resolve complex amendment cases, and safeguard the integrity of patient health information.
KeyDuties & Responsibilities
- Standardize amendment workflows across all care settings, ensuring consistency and compliance with enterprise policies and regulatory requirements.
- Review and process amendment requests for completeness, accuracy, and proper authorization; validate corrections in the EMR.
- Coordinate with compliance, risk management, HIM, clinical teams, and IT to execute complex amendments involving multiple systems or downstream applications.
- Develop, monitor and report amendment metrics, such as turnaround times and compliance rates; identify opportunities for process improvement.
- Provide education and training to staff on amendment policies, workflows, and regulatory requirements.
- Provides input on policies and SOPs related to medical record governance, amendments, and identity management and documentation integrity.
- Ensure compliance with HIPAA, state regulations, and organizational standards related to patient rights and documentation integrity.
- Engages in cross-functional projects to amendment requests and improve workflows.
- Participate in audits and investigations related to amendment requests and documentation integrity.
- Perform other duties as assigned, supporting enterprise data integrity initiatives.
Associate’s degree in Health Information Management (HIM), or related field required;
Bachelor’s or Master’s degree preferred. Five (5) years of progressive experience in HIM or clinical documentation, including experience demonstrating a graduated level of responsibility and scope, with at least three (3) years focused on medical record amendments or documentation integrity. Enterprise or multi-entity experience preferred.
RHIA, RHIT, or HIM-related certification preferred. Additional certifications such as CDIP or CHPS desirable.
Knowledge / Skills / Abilities- Strong understanding of clinical workflows and documentation standards.
- Ability to analyze and interpret medical record data for accuracy and completeness.
- Familiarity with EMR systems (Epic, Cerner) and amendment workflows.
- Knowledge of HIPAA Privacy/Security and regulatory requirements. Ability to interpret and apply regulatory requirements related to patient rights and documentation.
- Ability to be self-directed/self-motivated
- Must have excellent communication, interpersonal and collaboration skills across clinical and operational teams
- Detail-oriented with strong problem-solving and critical-thinking abilities.
- Strong analytical and problem-solving skills with ability to execute corrective action plans
- Ability to perform a variety of duties often changing from one task to another of a different nature without loss of efficiency or composure
