Job DescriptionTo support enterprise-wide efforts to ensure medical record data integrity across all care settings. To analyze, monitor, and remediate issues such as duplicate medical records, overlays, and incorrect patient identity linkages. The analyst collaborates with health information management (HIM), clinical informatics, clinical teams, registration, IT, and compliance to maintain accurate and reliable patient documentation. Responsibilities include conducting clinical reviews, facilitating and validating corrections, supporting policy adherence, and contributing to preventive strategies that enhance patient safety and data quality.
KeyDuties & Responsibilities
- Analyzes and monitors medical record integrity issues across EMR and paper records, including duplicates, overlays, erroneous merges, and other medical record data integrity issues.
- Supports remediation efforts by validating corrections and ensuring clinical documentation accuracy.
- Conducts clinical reviews to confirm patient-specific data corrections and appropriate use of amendments/addenda.
- Collaborates with HIM, clinical informatics, IT, compliance, registration, and clinical teams to resolve identity management issues and prevent recurrence.
- Assists in root-cause analysis for data integrity incidents and recommends process improvements.
- Develops, monitors and reports data integrity metrics, such as turnaround times and compliance rates; identifies opportunities for process improvement.
- Provides input on policies and SOPs related to medical record governance, amendments, and identity management and documentation integrity.
- Participates in training initiatives for clinical staff and registration teams on best practices for documentation integrity.
- Engages in cross-functional projects to reduce integrity defects and improve workflows.
- Maintains compliance with HIPAA, state regulations, and organizational standards for documentation integrity.
- Performs other duties as assigned, including audit support and special investigations related to patient identity and documentation.
Associate’s degree in Nursing, Health Information Management (HIM), or related clinical field required;
Bachelor’s or Master’s degree preferred. Five (5) years of progressive experience in clinical practice or HIM, including experience demonstrating a graduated level of responsibility and scope, with at least three (3) years focused on medical record integrity, patient identity management, or documentation analysis. Enterprise or multi-entity experience preferred.
Current State of Alabama licensure commensurate with healthcare/clinical degree (i.e. RN, etc.). RHIA, RHIT, or HIM-related certification preferred. Additional certifications such as CDIP or CHPS desirable.
Knowledge / Skills / Abilities- Strong understanding of clinical workflows and documentation standards.
- Ability to analyze and interpret medical record data for accuracy and completeness.
- Familiarity with EMR systems (Epic, Cerner). Knowledge of HIPAA Privacy/Security and regulatory requirements.
- Ability to be self-directed/self-motivated.
- Excellent communication, interpersonal and collaboration skills across clinical and operational teams.
- Detail-oriented with strong problem-solving and critical-thinking abilities.
- Strong analytical and problem-solving skills with ability to execute corrective action plans.
