Care Transition Coordinator

. We are looking for a passionate individual with sales and marketing skills in the healthcare world and experience working with elderly populations and families.

The Care Transition Coordinator will spend approximately 80% of the time in the market, generating and managing inquiries or referrals for assigned locations to position our company as the leader in services and provider of choice. They will create and implement an effective and measurable marketing plan involving key facility management personnel, act as liaison between the facility, the Admissions and Marketing Coordinator, and the referral community, monitor daily census results, adjust marketing plans accordingly, and manage the referral approval process according to the Company Commit to Care guidelines and protocols.

This position also coordinates the entire admission process and assists the family with administrative questions.

Responsibilities
  • Generate and manage inquiries or referrals for assigned locations.
  • Create and implement effective marketing plans.
  • Act as liaison between facilities, Admissions and Marketing Coordinator, and referral community.
  • Monitor daily census results and adjust marketing plans.
  • Manage the referral approval process following Company guidelines.
  • Coordinate admission process and assist families with administrative questions.
Qualifications
  • 2 years of successful admission process experience preferred.
  • Experience in long‑term care industry or managed care organizations required.
  • Community relations experience preferred.
  • Ability to anticipate the needs of discharge planners, case managers, physicians, referral contacts, family members, responsible parties, and facility staff.
  • Outgoing, high energy with superb verbal and written communication skills.
