The UAB Care Transitions Department includes more than 200 registered nurse care coordinators and social workers who are integral members of the interdisciplinary team.
Social workers provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on completing psychosocial assessments, providing resource referrals, and participating in care coordination and discharge planning for patients across the continuum of care. New graduates are supported by social work leaders who provide supervision as well as free monthly continuing education opportunities that help fulfill licensure requirements.
- Assist patients and their families in making full use of available national, state, institutional, community and pharmaceutical resources.
- Provide social evaluation, including placement of evaluation in medical record.
- Provide counseling to patients/families and/or groups.
- Participate in staffing, clinics and rounds in order to enhance team understanding of patient’s psychosocial situation.
- Facilitate discharge planning; arrange services such as home health, hospice, durable medical equipment and/or skilled nursing facility placement.
- Respond to the unique needs of the patient population as needed.
- Provide anticipatory, permanency and/or bereavement/loss counseling to patients and families and/or make appropriate community referrals.
- Supervise both bachelor’s and master’s level students.
- Act as a liaison to the community through public speaking.
- Master's degree in Social Work required. Work experience may NOT substitute for education requirement.
- For positions in The Kirklin Clinic (TKC), previous experience in a healthcare environment preferred.
- Currently licensed or eligible to be licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
- For positions in The Kirklin Clinic (TKC), license must be obtained within six months of employment.
