What Youâll Do
As a Care Navigator I, you will:
- Support patients transitioning between sites of care, ensuring their needs are met from hospital to home.
- Collaborate with clinical and administrative teams across the organization.
- Help develop postâacute care plans and perform routine patient checkâins.
- Distribute resources that remove care barriers and prevent avoidable readmissions.
- Advocate for patients by identifying needs such as prescriptions, equipment, and followâup services.
- Maintain strong proficiency with organizational systems, including EMR tools.
- Contribute to a psychologically safe environment where ideas are encouraged and valued.

What You Bring
- One (1) year of experience as a Medical Assistant, Paramedic, EMT, Military Allied Health Professional, Nursing Assistant, or similar healthcare role within value-based care or physician offices
. - Strong understanding of medical terminology and patient engagement.
- Excellent communication, critical thinking, and problemâsolving skills.
- Ability to work both independently and as part of a multiâdisciplinary team.
