Job Description

To lead processes to identify, triage, and remediate data integrity issues, such as duplicate medical records, overlays, erroneous merges, incorrectly linked identities, incorrect information carried forward, amendments, and other documentation errors. To drive preventive strategies, policy governance, education, and technology optimization. To direct a team that performs clinical and technical reviews, collaborates with Health Information Management (HIM), Patient Access/Registration, Clinical Operations, Risk Management, Compliance/Privacy, Legal, IT, and vendor partners, and serves as an enterprise subject‑matter expert for data integrity and interoperability.

Duties & Responsibilities
  • Provides enterprise oversight of medical record data integrity across electronic and paper records, ensuring completeness, accuracy, and reliability in all care settings.
  • Establishes and maintains standard operating procedures for identification, classification, remediation, documentation, and reporting of data integrity issues.
  • Oversees Master Patient Index (MPI) integrity processes, including detection and resolution of duplicates, overlays, false merges, incorrect linkages, and fragmented identities.
  • Facilitates root‑cause analysis and implements preventive controls through process improvements, staff training, and technology optimization.
  • Leads systematic analysis of incidents such as incorrect information carried forward, misfiled paper documents, wrong‑patient documentation, and amendments; coordinates corrective steps and ensures compliant documentation.
  • Oversees clinical reviews to validate patient‑specific data corrections, clinical content accuracy, and appropriate use of amendments and addenda.
  • Provides guidance for interfacing systems and upstream/downstream applications contributing data to the EMR, ensuring mapping fidelity and error‑handling workflows.
  • Authors and maintains policies related to medical record integrity, amendments, identity management, documentation standards, and interoperability; ensures compliance with HIPAA and applicable state and federal regulations.
  • Designs and delivers training for registration staff, clinical teams, HIM personnel, and IT analysts on data integrity practices, tools, and controls.
  • Partners with EMR vendors and third‑party solutions to optimize configurations, match algorithms, and work queues; defines business requirements for enhancements and participates in testing and validation.
  • Develops and publishes dashboards, KPIs, and trend analyses related to duplicate rates, overlay events, amendment turnaround time, and other integrity metrics; presents findings to leadership.
  • Recruits, mentors, and manages staff who execute data integrity reviews, perform chart corrections, manage work queues, and conduct clinical documentation assessments.
  • Leads cross‑functional projects to reduce integrity defects, streamline workflows, and enhance patient safety; benchmarks against industry best practices.
  • Performs other duties as assigned, including support for audits, accreditation surveys, and special investigations related to documentation integrity and patient identity.
Qualifications

Associate's degree in Health Information Management (HIM) or related field and three (3) years of experience as a HIM Supervisor or manager and five (5) years of experience working with an electronic medical record required. Bachelor's degree preferred. Must possess extensive knowledge of Federal and State laws pertaining to medical record integrity and identity management. Must be proficient in Microsoft applications.

Licenses

Active RHIA or RHIT certification required. CHPS, CPHIMS, CDIP, or CPCS preferred.
