Position Overview

  • Pay range: $24.05 - $39.10 / hour
  • Sign-on bonus: $7,500 sign-on bonus may be available
  • Shifts: weekends AM and PM

The UAB Care Transitions Department includes more than 200 registered nurse care coordinators and social workers who are integral members of the interdisciplinary team. Social workers provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on completing psychosocial assessments, providing resource referrals, and participating in care coordination and discharge planning for patients across the continuum of care.

Job Description
  • To provide individuals, families and groups with the psychosocial support needed to cope with chronic, acute or terminal illnesses.
  • To provide services including advising family caregivers, providing patient education and counseling, and making referrals for other services.
  • May also provide care and case management or interventions designed to promote health, prevent disease and address barriers to access to healthcare.
  • To facilitate the discharge planning process and identify community resources to assist patients and their families.
  • To act as a clinical resource to both social service students and the community.

Most positions require 4 weeks of orientation, minimally, up to 7 weeks, based on previous experience. Orientation is completed Monday-Friday with a dedicated preceptor.

Qualifications
  • Master's degree in Social Work required. Work experience may NOT substitute for education requirement.
  • Currently licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
  • For units/departments other than UAB Hospital, certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment.
  • If job is in UAB Hospital, successful completion of First Five Rapid Response Basics online training required within 30 days of hire into position and must be maintained annually.
