Social workers provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on completing psychosocial assessments, providing resource referrals, and participating in care coordination and discharge planning for patients across the continuum of care. Job Description:
- Provide psychosocial support to individuals, families, and groups coping with chronic, acute, or terminal illnesses.
- Advise family caregivers, educate patients, provide counseling, and make referrals for additional services.
- Provide case management and interventions to promote health, prevent disease, and address barriers to healthcare access.
- Facilitate the discharge planning process and identify community resources for patients and families.
- Act as a clinical resource to social service students and the community.
Qualifications:
- Master's degree in Social Work (required).
- Licensed Master Social Worker (LMSW) credential by the Alabama State Board of Social Work Examiners (required).
- For units/departments outside UAB Hospital, Basic Life Support (BLS) certification from AHA (required or to be obtained within 30 days of employment).
- For UAB Hospital positions, completion of the First Five Rapid Response Basics online training within 30 days of hire, maintained annually.
