We are looking for an enthusiastic social worker to join our dynamic Adults Multi-Disciplinary Team on a locum basis for three months. What you will do:

  • Work as part of an integrated and supportive team, ensuring effective planning, monitoring, and evaluation of work within the Multi-Disciplinary Team.
  • Adhere to Social Work England standards, providing social work support and promoting autonomy and choice among adults and carers.
  • Conduct needs-led assessments, develop care and support plans, and initiate multi-disciplinary assessments where necessary.
  • Provide support to adults, their carers, and families, focusing on preventing unnecessary hospital admissions.
  • Operate in line with the Professional Capabilities Framework and uphold the principles of wellbeing and rights.
  • Communicate complex information to stakeholders sensitively and effectively.
  • Participate in safeguarding enquiries and interagency meetings, ensuring the best outcomes for those at risk.
