About this job- Act as an autonomous practitioner in accordance with and upholding the NMC Code of Conduct and Scope of Professional Practise for Nurses, Midwives and Health Visitors, providing a holistic approach to health by completing a full assessment of the physical, social, emotional and environmental needs of people with long term conditions and frailty.
- Be pro-active in identifying health needs in this client group.
- Advise and inform the patients and their carers about health issues to support a greater sense of self-reliance and independence.
