Community Staff Nurse

Responsibilities

• Complete a full assessment of the physical, social, emotional, and environmental needs of people with long-term conditions and frailty.
• Be pro‑active in identifying health needs in this client group.
• Advise and inform patients and their carers about health issues to support greater self‑reliance and independence.
• Work shifts across the 7 days; access to a car for work purposes is essential.
• Work in a lone capacity under the direction of the Clinical Team Leader/Caseload Manager while maintaining clinical standards using evidence‑based practice.

• Good communication and interpersonal skills; adaptable, reliable and approachable.
• Sound knowledge of recent changes within the NHS and current professional issues.
• Understanding of frailty and long-term condition management.
• Knowledge of Clinical Governance, Best Practice, Risk Management, Clinical Audit and relevant Professional Codes.
• English language ability as required by Part 7 of the Immigration Act 2016.

• Priority given to staff on the Trust's Redeployment Register.
• Flexible working arrangements are an important part of the role.

The Trust pays staff in line with national NHS Terms and Conditions; salary offers may consider relevant experience and are pro‑rata for part‑time positions.
