- Schedule:
Typically 8:00 AM - 4:30 PM, Monday - Friday, with flexibility to create your own daily schedule based on patient load. - On-Call Rotation:
Typically once a week. - Weekend Rotation:
Typically one weekend a month.

DescriptionProvides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Education / LicenseDiploma from an accredited school/college of nursing. Required professional licensure at time of hire.
Responsibilities- Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.
- Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Carries out physician and nursing orders. Attends team meetings and care plan meetings.
- Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.
- Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.
Other duties as assigned.
