** Position Overview
** United Health Services is seeking a Physical Therapist to join our compassionate and collaborative Home Care team. In this role, you’ll provide one-on-one therapy to patients in their homes—helping them regain strength, improve mobility, and maintain independence in a familiar environment. You’ll be part of a skilled, interdisciplinary team that shares information, supports one another, and puts patient goals first.

* * Compensation Range:**$33.57 - $51.39 per hour, depending on experience-----#
** Job Responsibilities
*** Assess patient strengths, limitations, and needs using standardized measurement, prediction, and evaluation tests
* Develop and implement individualized short- and long-term treatment goals based on patient assessments
* Administer and interpret clinical physical therapy services, adjusting treatment plans as needed based on patient response
* Maintain accurate and timely patient treatment records, documenting progress and changes in electronic health systems
* Ensure all documentation meets requirements of reimbursement and regulatory agencies, completed within two business days
* Establish and maintain effective working relationships with patients, families, and interdisciplinary team members
* Participate in patient care conferences and collaborate with other healthcare professionals for coordinated care
* Provide education to patients and families regarding treatment plans, safety, and self-care techniques
* Participate in discharge planning and make appropriate referrals, coordinating with vendors for durable medical equipment
* Supervise and provide direction to Physical Therapist Assistants during treatment sessions
* Maintain licensure requirements and pursue continuing education to stay current with professional standards
* Engage in departmental and organizational performance improvement initiatives to enhance quality of care#
** Position Qualifications
**** Minimum

Required:

*** Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York
* Graduate of an accredited school of physical therapy with a Bachelor's or Master’s Degree or equivalent in physical therapy
* Current CPR certification
* Previous experience in a licensed healthcare setting
** Preferred:
*** Master’s or Doctorate Degree in Physical Therapy
* Previous physical therapy experience in a hospital-based setting-----#
