Care Manager Level 2 — Protective Services for Adults

Grade 8 | $19.95–$22.45/hr |
FLSA Non-Exempt

Qualifications
  • Education
  • Minimum associate degree in human services or related field.
  • Bachelor’s degree in a human services field is preferred.
  • Experience
  • Minimum of one year experience for bachelor’s degree and two years for associate degree working in providing direct social work service to special needs individuals including those living with a mental illness, developmental disability, substance use or elderly.
  • Knowledge of community mental health services and services available to developmentally disabled and geriatric.
  • Skills
    • Good computer skills with working knowledge of Microsoft Office necessary.
    • Knowledge of special needs individuals including mental illness/psychiatric disorders/developmentally disabled and geriatrics.
Responsibilities
  • Excellent communication skills, both oral and written.
  • High degree of organizational skills to meet assigned work demands.
  • Demonstrated ability to work with a team as well as function independently.
  • Good computer skills with working knowledge of Microsoft Office necessary.
  • Knowledge of special needs individuals including mental illness/psychiatric disorders/developmentally disabled and geriatrics.
  • The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. or less.
  • A valid driver’s license and the ability to legally operate a vehicle in NYS.
  • Regular and substantial contact with those individuals receiving services.
  • Assist recipients in learning to use financial resources responsibly, including assistance with budgets.
  • Provide crisis intervention services as necessary.
  • Promote self‑advocacy and personal empowerment.
  • Maintain case records, progress notes and assessments, as required in compliance with Agency, County and Office of Children and Family Services regulations.
  • May carry caseload of up to 40 recipients and provide a minimum of one visit per month.
  • Complete and submit monthly reports related to services provided and individuals served.
  • Encourage medication, education and compliance.
  • Communicate regularly with other providers and individuals to develop or enhance consistency of services.
  • Coordinates Representative Payee services as needed.
  • Participate in ongoing training, attend meetings as required.
  • Physical condition commensurate with the demands of the position.
  • Perform other related duties as required.
