Grade 8 | $19.95–$22.45/hr |

- Education
- Minimum associate degree in human services or related field.
- Bachelor’s degree in a human services field is preferred.
- Experience
- Minimum of one year experience for bachelor’s degree and two years for associate degree working in providing direct social work service to special needs individuals including those living with a mental illness, developmental disability, substance use or elderly.
- Knowledge of community mental health services and services available to developmentally disabled and geriatric.
- Skills
- Good computer skills with working knowledge of Microsoft Office necessary.
- Knowledge of special needs individuals including mental illness/psychiatric disorders/developmentally disabled and geriatrics.
- Excellent communication skills, both oral and written.
- High degree of organizational skills to meet assigned work demands.
- Demonstrated ability to work with a team as well as function independently.
- Good computer skills with working knowledge of Microsoft Office necessary.
- Knowledge of special needs individuals including mental illness/psychiatric disorders/developmentally disabled and geriatrics.
- The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs. or less.
- A valid driver’s license and the ability to legally operate a vehicle in NYS.
- Regular and substantial contact with those individuals receiving services.
- Assist recipients in learning to use financial resources responsibly, including assistance with budgets.
- Provide crisis intervention services as necessary.
- Promote self‑advocacy and personal empowerment.
- Maintain case records, progress notes and assessments, as required in compliance with Agency, County and Office of Children and Family Services regulations.
- May carry caseload of up to 40 recipients and provide a minimum of one visit per month.
- Complete and submit monthly reports related to services provided and individuals served.
- Encourage medication, education and compliance.
- Communicate regularly with other providers and individuals to develop or enhance consistency of services.
- Coordinates Representative Payee services as needed.
- Participate in ongoing training, attend meetings as required.
- Physical condition commensurate with the demands of the position.
- Perform other related duties as required.
