• Provide skilled nursing care to senior clients in their homes, focusing on delivering exceptional patient outcomes and improving their quality of life

• Conduct initial and ongoing assessments of clients' physical and emotional needs, developing personalized care plans to address their unique requirements

• Administer medications, perform wound care, and manage chronic conditions, as ordered by physicians and in accordance with established protocols

• Collaborate with other healthcare professionals, including physicians, therapists, and aides, to coordinate client care and ensure continuity of services

• Document all care activities, medications, and client progress in a thorough and timely manner

• Maintain accurate and up-to-date records, adhering to HIPAA guidelines and organizational policies

• Current LPN licensure in the State of Montana

• High school diploma or equivalent required; some college or associate's degree in nursing or a related field preferred

• Minimum 0 years of experience as a LPN; new graduates welcome to apply

• Ability to lift 25 pounds and stand for extended periods

• Strong communication, interpersonal, and organizational skills
