Nursing home social workers provide vital support and advocacy for elderly residents to improve their quality of life.
- Assessment and Care Planning:
Conduct assessments to identify residents' psychosocial, mental, and emotional needs; develop individualized care plans with healthcare teams. - Advocacy and Support:
Advocate for residents' rights, offer emotional support and counseling to residents and families, and help navigate long-term care challenges. - Resource Coordination:
Connect residents and families with community resources, mental health services, rehabilitation, and discharge planning. - Education and Training:
Educate staff on residents' psychosocial needs, cultural diversity, and prevention of abuse and neglect. - Group Facilitation:
Organize and lead group activities to promote social interaction and emotional well-being. - Documentation:
Maintain accurate records of residents' progress and interventions, ensuring compliance with regulations.
This part-time role offers $33.05 - $39.80 per hour with a minimum of 15 hours weekly, requiring in-person work.
