• Undertake functional and holistic occupational therapy assessments in community settings.
• Deliver rehabilitation-focused interventions aimed at promoting independence.
• Support hospital discharge processes and admission avoidance pathways.
• Recommend and prescribe equipment and minor adaptations to facilitate day-to-day activities.
• Complete mental capacity assessments and contribute to best interest decisions.
• Work collaboratively with social workers, health colleagues, reablement teams, and partner agencies.
• Maintain accurate case records in line with council policies and statutory requirements.

• You must have community-based occupational therapy experience with a focus on rehabilitation.
• Proven ability to manage a varied and fast-paced caseload effectively.
• Access to a car and the ability to travel across the locality with confidence.
• Must be registered with the Health and Care Professions Council HCPC.
