Position: Day Program Case Manager
Locations: Wakefield, MA | Woburn, MA | Beverly, MA
Pay Rate: $22.00 per hour
Hours: Full-time, Monday - Friday (8am-4pm)

The Day Program Case Manager plays a vital role in our services by building and maintaining professional relationships with individuals with developmental disabilities and their interdisciplinary teams to ensure coordinated support.
- Collaborate with departments, individuals, families, residences, and funding sources to meet individual needs and promote choices
- Conduct assessments, evaluations, and re-evaluations to determine support levels and support planning
- Monitor and evaluate the effectiveness of support plans, providing guidance on documentation
- Participate in planning meetings, advocating for individuals’ needs and contributing to department and individual goals
- Provide personal care and assistance, supporting individuals to reach their goals, and cover group activities as needed
- High School diploma required; BA in Human Services or related field preferred
- At least 1 year supporting adults with developmental disabilities preferred
- Passion and dedication to supporting people
- Valid driver’s license, reliable transportation, and a clean driving record
- Ability to pass background and reference checks
