, we’re growing and looking for a Facility Success Coordinator (FT) to support our facility-facing operations across post-acute and long-term care settings in Southern California.

hybrid, full-time role is focused on maintaining strong relationships with assigned facilities through routine check-ins, on‑site presence, and administrative coordination—ensuring smooth program execution while partnering closely with our internal clinical, operations, and technology teams.

• Conduct routine check-ins with assigned facilities to support communication and visibility
• Serve as a facility-facing point of contact for day‑to‑day coordination and follow‑up
• Observe device usage remotely and escalate needs or issues to internal teams
• Support facility onboarding tasks, administrative workflows, and checklist completion
• Assist with tracking and coordinating consent‑related processes
• Communicate updates, questions, and observations to internal stakeholders
• Visit assigned facilities in Southern California ~4 times per month

• Experience in coordination, customer success, operations, or healthcare administration
• Strong communication, organization, and follow‑up skills
• Ability to manage multiple tasks and priorities
• Experience with post‑acute or long‑term care facilities is a plus
• Familiarity with technology‑enabled healthcare solutions is a plus
