Position: START Coordinator II - Crisis Services

POSITION SUMMARY:

Under the supervision of the START Team Leader, the START Coordinator II is responsible for providing crisis support, linkages, outreach, and consultation services for individuals with a developmental disability and behavioral health needs.

Responsibilities
  • Works with individuals’ system of support and provides systemic consultation.
  • Collaborates with external partners and internal resources to develop services that are person-centered and meet individuals’ needs.
  • Prepares agendas and documents outcomes for individual mentoring sessions held with Center for START Services.
  • Maintains linkages and relationships with community partners.
  • Assists with referrals for consultation and treatment as needed.
  • Provides outreach visits to the individuals’ homes, day, and vocational settings as needed.
  • Uses the START Action Plan for case planning and strategizing.
  • Develops Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans for Resource Center admissions or inpatient hospital admissions and any other applicable documentation of services provided.
  • Maintains an active caseload of individuals referred for START Services (variability occurs depending on activity level of cases and the project scope).
  • Ensures the coordination of support meetings and crisis plan development for individuals served through START.
  • Participates in recurring meetings with START leadership, clinical team, and Resource Center program.
  • Participates in National Center for START Services trainings and events.
  • Participates in the START Network, which brings together projects from across North America through an online Resource Center, conference calls, select study groups and a meeting at an annual conference as recommended.
  • Addresses emergency situations as they occur in the program according to established policies and procedures, and ensure communication of such situations in a timely fashion.
  • Maintains on-call responsibilities and supports alternate locations, as required.
  • Adheres to flexible schedule that reflects the needs of the program(s).
  • Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.

Qualifications
  • Education:

    Master’s Degree in Social Work, Psychology, Counseling, or other human service field required
  • Experience:

    Experience working with people who have IDD, mental health and/or behavioral health needs preferred
  • Specialized Training, Licenses,

    Certifications:

    START Coordinator II certification must be completed within 12 months (may be 18 months for part-time coordinators)
  • Knowledge, Skills, Competencies, and Abilities:
    Ability to work with complex systems and an interest in the population is essential;
    Independent, organized, and effective oral and written communication are necessary
