- Supports a caseload of assigned individuals in accordance with CTI model standards.
- Uses personal lived experience to engage and inspire individuals through trust, mutual understanding, and shared perspective.
- Conducts assertive outreach and engagement efforts in both hospital and community settings.
- Participates in pre-discharge engagement and assist in development of transition and aftercare plans.
- Assists individuals with accessing housing, medical and behavioral health care, public benefits, employment supports, and community resources.
- Provides coaching and role modeling around independent living, self-advocacy, and coping strategies.
- Accompanies individuals to appointments and community activities as needed.
- Collaborates with hospital teams, community providers, and natural supports to strengthen care continuity.
- Participates in weekly team meetings, case reviews, and model fidelity training.
- Maintains accurate, timely documentation in the electronic health record, reflecting the intensity and focus of services provided through each CTI phase.
- Supports individuals through crisis episodes and assists in developing relapse prevention and crisis plans.
- Assists individuals in preparing for final transition of supports and program graduation.
- Ensures that all required documentation (paper and electronic) is completed/processed appropriately, in a timely manner, and meets quality and compliance thresholds.
- Performs other duties as assigned.
- Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
- Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
- Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
- Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
- Meets all job requirements and stays in compliance.
- Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.

Qualifications- Education:
High school diploma or equivalent required - Experience:
Experience working with individuals with co-occurring disorders such as mental health, substance use, or I/DD preferred. Familiarity with community mental health services, benefits systems, and care coordination preferred. - Specialized Training, Licenses,
Certifications:
NYS Peer Specialist Certification required. Qualified candidates without a NYS Peer Specialist Certification may be considered provided they sign an education agreement and obtain certification within 4 months of hire. Complete the CTI certification process within 12 months of hiring. Complete 9.58 designation certification within 6 months of hiring. - Knowledge, Skills, Competencies, and Abilities:
Strong interpersonal and communication skills. Ability to use lived experience in a supportive and professional manner. Comfort working in homes, hospitals, and diverse community settings. Understanding of trauma-informed, culturally competent, and recovery-oriented practices. Computer skills and ability to learn electronic health records and related applications. - Other Requirements:
Valid driver's license and satisfy criteria to be an agency driver required. A current PPD (tuberculosis) test or equivalent screening (will be administered upon hire if needed). A physical examination completed within the last year by a licensed healthcare provider.
