Position
Purpose:
The Navigator Supervisor & Resource Coordinator serves as a clinical-informed care coordination and community systems lead for the BPHN Healthcare Navigators. This role ensures Black residents of Montgomery County experience equitable, timely, culturally responsive access to healthcare, insurance navigation, and community-based support services. This role provides clinical oversight and systems guidance to Healthcare Navigators, manages referral workflows, builds cross-sector partnerships, and ensures performance and quality standards align with BPHN’s mission to reduce health disparities and improve health outcomes in the Black community of Montgomery County.
Essential Duties and Responsibilities:- Serve as the clinical and systems lead for client referral intake, eligibility review, and service assignment, ensuring referrals are assessed within 48 business hours.
- Act as a liaison between BPHN clients, healthcare providers, insurers, and community-based organizations to advocate for client needs, treatment access, and continuity of care.
- Support trauma-informed, culturally responsive, and client-centered engagement practices across all service interactions.
- Provide supervision/guidance, quality assurance, and technical support to Healthcare Navigators to ensure alignment with their duties and responsibilities of their role.
- Review service documentation and referral workflows to ensure timely, accurate, and compliant data entry in Salesforce by the Healthcare Navigators.
- Develop and maintain a countywide network of healthcare resources, social service agencies, faith-based organizations, and community partners to expand access to care and resources for Black residents of Montgomery County.
- Represent BPHN in community forums, outreach events, and cross-sector planning initiatives focused on health equity, access, and community wellness.
- Demonstrate advanced working knowledge of public and private health insurance options;
Medicaid/Medicare eligibility and enrollment pathways; and health coverage appeals and access barriers. - Provide coaching and case consultation to Healthcare Navigators supporting clients with complex health and social needs.
- Complete certification and coursework for Community Health Worker (CHW) classification within one year of hire (if not already credentialed).
- Monitor and ensure there is a 90% utilization rate of BPHN Clients with BPHN Healthcare Providers by referrals submitted and completed by Healthcare Navigators to ensure timely access to care.
- Establish a network of community resources, partners, and organizations to help provide education, outreach, and access in order to improve outcomes and enhance equity for the Black residents of Montgomery County.
- Coordinate and organize logistics and planning of a variety of community events with the BPHN Navigator team and local community organizations to expand access and outreach to the Black communities throughout Montgomery County regarding their overall health needs.
- 90% of BPHN clients receive a response to their referral within one (1) business day.
- 100% of Navigators compliance with timely and accurate documentation in Salesforce.
- 85% customer engagement rate.
- Establish network of at least three partnerships/organizations in each district within Montgomery County.
- Essential Duties and Responsibilities:
- Contributes to agency resource development efforts, as appropriate.
- Participates in all mandatory meetings and trainings as required by NCCF.
- Represents the NCCF in external policy and program arenas, as approved.
- Presents and publishes professionally on topics related to specialty service.
- Master’s degree in Social Work (while licensed at the clinical level) is preferred or in closely related field such as Public Health.
- Must have a vehicle and a valid driver’s license.
- Solid judgement/temperament in dealing with confidential and sensitive information and issues.
- Commitment to teamwork, collaboration, and the agency’s mission to progressively serve vulnerable children and their families.
While performing the responsibilities of the job, the contractor is required to talk and hear. The contractor is often required to sit and use their hands and fingers to handle or feel. The contractor is occasionally required to stand, walk, reach with arms and hands, climb or balance, and stoop, kneel, crouch or crawl. Vision abilities required by this job require close vision working with a computer monitor.
While performing the duties of this job, the contractor is occasionally exposed to moving mechanical parts and vehicles. The noise level in the work environment is usually quiet to moderate.
