This is a grant funded position. The Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This position may require village travel.
The educator will order labs for patients according to hospital policies and document education in the electronic health record. The educator will assist in planning, coordinating, and providing diabetes-related health promotion activities in the YK Delta Region. Patients and activities are accessed via multiple village trips per month. Educator will work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care.
Educator assists the Diabetes Coordinator with maintenance of the Diabetes Registry and grant requirements. Educator works closely with the Registered Dietitian, Clinical Diabetes Educator, and Diabetes Pharmacist to ensure comprehensive care of clients. Educator participates in program outreach activities such as monthly Diabetes Support Groups, radio shows, media campaigns, and special events. Educator participates in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders.
Educator believes in and promotes Napartet and all corporate endeavors. Performs other duties as assigned or required.
- Provide comprehensive diabetes education and care in clinic settings and through telecommunication methods.
- Order labs for patients according to hospital policies and document education in the electronic health record.
- Plan, coordinate, and provide diabetes-related health promotion activities in the YK Delta Region.
- Collaborate with village clinics, hospital staff, Diabetes Coordinator, and other care team members to promote diabetes standards of care.
- Participate in program outreach activities (e.g., Diabetes Support Groups, radio shows, media campaigns, special events).
- Assist in maintenance of the Diabetes Registry and grant reporting requirements.
- Support care coordination with the Registered Dietitian and Diabetes Pharmacist to ensure comprehensive client care.
- Engage in department planning and goal setting to improve patient care and partnerships with tribal leaders.
- Promote Napartet and corporate endeavors as applicable. Perform other duties as assigned.
- Must have completed a program of education in nursing from an NLN approved institution.
- Minimum Associate's Degree in Nursing required.
- BSN or MSN preferred.
- New graduates (with relevant license) may be considered.
- Two (2) years experience in a clinical health setting preferred.
- Demonstrated experience collaborating with a variety of disciplines for project completion.
- Background in Diabetes Education preferred.
- Valid license in good standing in the State of Alaska as a Registered Professional Nurse.
- Must obtain valid CDCES (Certified Diabetes Care and Education Specialist) certification within 2 years.
- Preferred Basic Life Support (BLS) certification.
- Work in a typical office, hospital, and village clinic setting.
- Comprehensive knowledge of PC computers.
- Ability to operate diabetes care medical equipment (glucose meter, height/weight scales, etc.).
