Overview
The Clinic Care Manager will support and assist providers with daily tasks, coordinate services for patients, manage specialty referrals, support patients in care, and identify relevant resources. The role involves collaboration with outside providers to ensure seamless transitions of care, patient education, and access to applicable resources. The position balances administrative duties with working knowledge of Medical Assistant duties to promote timely access to care, comprehensive and continuous care, and enhanced patient well-being.
PositionQualifications
- Education: High school or GED required; associate degree or related certification preferred (e.g., CMA, NCMA or working toward a degree).
- Experience: A minimum of two (2) years of experience working in the health setting.
- Licenses,
Certifications:
CPR or First Aid required within 6 months of hire. - Specialized
Skills:
Working knowledge in the medical field. - Preferred Qualifications: One to two years of experience working in case management or related area.
- Record patient history and personal information.
- Measure vital signs (e.g., blood pressure) and record them appropriately.
- Assist providers with patient examinations.
- Administer injections or medications as directed by the provider and as permitted by state law.
- Schedule patient appointments.
- Draw and prepare blood or other bodily fluids for laboratory tests and send outs.
- Enter patient information into the EHR.
- Direct patient care to ensure needs are met and clinical policy is followed.
- Update and revise patient healthcare plans as needed.
- Demonstrate knowledge in medical care, including diagnostic procedures, medications, symptoms, and other treatment-related therapies.
- Evaluate and determine patient needs.
- Review and evaluate the effectiveness of individualized care plans.
- Consult with providers to collaborate in patient care and performance improvement activities.
- Create care plans with objectives, goals, and actions to meet patient needs.
- Implement and monitor care plans to ensure effectiveness and appropriateness of services.
- Provide referrals to appropriate community resources and other healthcare facilities; facilitate access and communication when multiple services are involved; coordinate services to avoid duplication and ensure needs are met.
- Refer to a provider any patient whose health status has declined and follow up on referrals.
- Provide patient consultations and clinically assess medication adherence.
- Administer nursing care as ordered by the provider.
- Medical Assistant duties as necessary.
- Type reports, memos, letters, and other documents using word processing software.
- Perform general office duties such as ordering medications and maintaining records management databases.
- File and retrieve medical documents, records, and reports.
- Make travel arrangements as needed.
