Summary
The Community-Based Mental Health Therapist provides person-centered, recovery-oriented therapy services to clients with moderate to higher levels of need. Services are delivered in the home and other community locations, bringing care to our clients in their natural environments, but may also be provided in office or via telehealth, as appropriate.
As part of an interdisciplinary care team, the therapist collaborates with behavioral health colleagues as well as medical, dental, and SA/DV providers to address the whole-person needs of clients. This role includes supporting continuity of care across service levels, helping clients maintain progress when transitioning to or from more intensive services such as Community Support Team (CST) or other higher levels of care.
Therapy services are guided by evidence-based models and are responsive to the cultural, social, and trauma-related needs of the community.
Essential Duties and Responsibilities Provides individual, family, and/or group therapy to children, adolescents, and adults with moderate to higher acuity needs. Provides an average of 5-6 hours of direct client service per workday to ensure community access to care and meet established performance standards.Supports clients in stepping up to CST and other higher levels of care and stepping down to lower levels of service, ensuring smooth transitions and continuity of care.Engages clients proactively and assertively, utilizing outreach, follow-up, and flexible service delivery to reduce barriers to care, maintain consistent engagement, and support clients in community-based settings.Participates on inter-disciplinary teams of behavioral health providers including therapists, case managers/care coordinators, peer support specialists, and other providers indicated to serve individual clients.Provides post-crisis stabilization and follow-up services for clients following a crisis event and/or hospitalization.Completes clinical assessments to formulate an accurate diagnosis and comprehensive treatment interventions to serve the identified client and family.Provides services that reflect person-centered and family-centered, recovery-oriented care, attending to the cultural and social needs of each client and family.Learns and implements time-limited evidence-based practices as determined by client need and PCH standards of care, which may include Interpersonal Therapy (IPT), Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), Dialectical Behavior Therapy (DBT), and Cognitive Behavioral Therapy for Suicide Prevention (CBT-Suicide).In collaboration with clients/families and inter-disciplinary care teams, determines frequency, duration, and intensity of treatment, and develops actionable treatment plans.Assesses and stabilizes high risk individuals and/or families.Collaborates in the provision of case management and care coordination services as defined in the service plan that coordinate, support, and advocate for clients who need multiple services and require assistance in gaining access to and in using mental health, social, vocational, educational, child welfare, and other community services and resources.Completes clinical documentation in accordance with Behavioral Health and organizational policies and procedures, established time frames, and meeting quality standards.Partners with integrated care teams across the organization – including medical, dental, and SA/DV – to support whole-person care.Coordinates with other community agencies and external systems such as DCFS, schools, hospitals, other providers.Responds to clients and/or family needs outside of regularly scheduled session times, as needed.Provides mission-focused care towards improving the health of our communities and empowering individuals through compassionate, whole-person care. Embodies the values of PCH, including access, community, diversity, inclusivity, and respect, and service excellence, in all work operations.Participates in team meetings, case reviews, supervision, in-service and other trainings, and consultations.Perform other duties as assigned.Qualifications

Education and Experience - Masters degree in Counseling, Psychology, Clinical Social Work, Marriage and Family Therapy or related field required.
- Experience providing therapy, assessments, and treatment planning with children, adolescents, and/or adults living with mental illness required.
- Experience working with moderate- to high-acuity clients strongly preferred.
- IM+CANS familiarity preferred.
- Capacity to work independently while collaborating as part of a team is essential.
