• Assess the physical, functional, psychosocial and nutritional status of assigned patients in a systematic and continuous manner to provide required patient services.
• Professionally consult with physicians, other team members, patients/families, and other caregivers to formulate a plan of care and coordinate delivery of services.
• Provide nursing services in accordance with the plan of care and utilizing the nursing processes.
• Prepare clinical documentation for bi‑weekly interdisciplinary meetings to reflect patient’s current status. Informs the physician and other IDT members of changes in the patient’s conditions and communicates recommended interventions.
• Participate in service line meetings/team conferences to ensure the coordination of all patient/family services and prioritization of their needs are being met as required.

• Prior hospice or home health care experience preferred.
• LPN licensed in the state of Nebraska or compact state required.
• Three years’ experience as a nurse required.
• Valid state driver’s license, current automobile insurance and reliability.
• Average to above average computer skills.
• Must be compassionate and able to work independently.
