Requirements: Current unrestricted AR RN license, BLS certification, strong assessment skills with critical thinking and high-level problem solving within a healthcare facility. Excellent documentation and communication skills, with the ability to stay calm and cope with human suffering/emergencies. Prefer hospice experience, home health, or acute care nursing experience with computer charting. Position requires use of personal vehicle, a valid driver's license, and current auto insurance.
General Description: The Liaison is an integral part of the Interdisciplinary Team and works closely with the Medical Director. The primary responsibilities include referral, coordination, and execution of the admission process into Hospice care. The nurse must effectively communicate the Hospice concept to families and patients, develop the plan of care, and communicate patient/family needs to the team. Must be able to train and educate patients/caregivers, establish appropriate discipline frequency, and handle human suffering professionally.
Flexibility to change schedules at short notice and daily travel are required. The nurse should possess excellent clinical skills, be able to identify problems, perform head-to-toe assessments, educate and consult with physicians, and execute orders to meet patient needs. Assistance with admissions is also part of the role.
