RN - Hospice

Monday to Friday, 8 AM - 4 PM, with some weekend and on-call requirements.

The Home Health Nurse will utilize the nursing process to assess patient needs and provide quality care in accordance with evidence-based practice, state and federal regulations, and agency policies. The position requires effective case management, OASIS documentation, and coordination of interdisciplinary care for patients.

• Conduct comprehensive assessments of patients and care givers.
• Develop and communicate individualized plans of care.
• Manage case loads and maintain productivity standards.
• Ensure timely and accurate OASIS documentation.
• Implement nursing interventions according to established care plans.
• Educate patients and care givers on treatment options and care instructions.
• Coordinate care with other healthcare professionals and services.
• Participate in interdisciplinary team meetings and case conferences.

• Experience in home health care preferred.
• Knowledge of OASIS documentation standards.
• Ability to manage high-tech nursing skills including IV therapy and wound care.
