** - Utilize the nursing process to assess patient and caregiver needs, providing quality care aligned with evidence-based practices and regulations.

- Develop and communicate a personalized plan of care for each patient, ensuring active involvement of patients and caregivers in the process.

- Serve as a dedicated Case Manager for assigned patients, effectively managing caseloads and visit schedules while meeting agency productivity standards.

- Complete and maintain accurate OASIS documentation in accordance with agency policies, state and federal regulations, ensuring timely and up-to-date records.

- Implement medical plans, nursing interventions, and necessary procedures to deliver safe and effective care, adhering to evidence-based practices and regulations.

- Stay proficient in a variety of high-tech skills, including IV maintenance, wound VAC, and complex wound care, while upholding infection control standards.

- Collaborate with LPNs within their scope of practice, ensuring adherence to care plans and delegating tasks to meet patient needs.

- Evaluate multiple aspects of the care plan, including identified problems and patient responses, to ensure high-quality care delivery.

- Maintain precise documentation of nursing care for integration into the healthcare team’s information flow, supporting ongoing quality of care.

- Follow agency protocols and processes for visit management to ensure effective and timely patient care.

- Educate patients and caregivers regarding treatment options, medication side effects, discharge instructions, and other crucial information.

- Identify interdisciplinary needs for patient care and communicate timely to other disciplines.

- Complete necessary agency, state, and federal forms accurately and promptly during patient visits.

- Conduct visits efficiently and communicate visit information to the Scheduler as needed.

- Assess patients' needs for Home Care Aide services, developing coordinated plans of care while providing ongoing supervision.

- Report patient incidents such as infections or injuries to the QI Specialist promptly.

- Coordinate safe patient discharges or transfers, ensuring all paperwork and patient information is processed correctly.

- Collaborate with physicians, providers, and team members to support the recommended plan of care for safe and effective patient family care.

- Participate in monthly peer review audits, providing and accepting constructive feedback professionally.

- Schedule and facilitate case conferences with team members to enhance communication and ensure timely documentation.

- Advocate for patient care issues to enhance the quality and appropriateness of care for each patient and family.

- Actively participate in IDT meetings to develop an individualized plan of care within established time frames.

- Continuously update knowledge on pain control, symptom management, and high-tech skills while ensuring infection control standards.
