Registered Nurse (RN) – Annual Wellness Visit Nurse

This position performs primary care annual wellness exams, following guidelines and meeting documentation requirements. The nurse also performs outreach as necessary, supports the development of annual wellness exam protocols and standing orders.

Travel to off‑site offices is required.

$82,000.00 per year – $92,000.00 per year

Assist and support in the coordination, development, implementation, and evaluation of the standards and practice of Primary Care Annual Wellness Exams.

• Graduation from an accredited school of nursing (required). Matriculation into a Bachelor's Degree in Nursing preferred.
• Five years of clinical experience required, with at least one year in a medical office setting preferred.
• Valid NY and VT Nursing license required.
• BLS required.
