Status

Full Time (salaried‑exempt) 40 hours per week.

Hours:

8:00 AM to 5:00 PM, combination of weekdays and weekends. Work outside of these hours may be required.

Location:

Working all throughout Central Oregon.

Summary/Objective

The Nurse Practitioner will be responsible for performing the face‑to‑face visits as required by Medicare’s final ruling on Section 3132(b) of PPACA (the Affordable Care Act), updating Sec 418.22 Certification of terminal illness, and adhering to all State, Federal, and compliance regulations, as well as other provider responsibilities not requiring a physician to perform.

Essential Functions
  • Responsible for reviewing the status of the hospice patient with the IDT prior to conducting the required face‑to‑face visit as designated by the Hospice Medical Director.
  • Conducts a thorough, comprehensive assessment of the patient in order to support evidence of decline and a continued need for hospice if appropriate.
  • Completes appropriate documentation within 48 hours of visit.
  • Executes e‑scribe orders in a timely manner.
  • Assists in the completion of death certificates in OVERS.
  • Provides education to clinical and other staff.
  • Mentors and serves as a clinical resource for hospice field RNs.
  • Collaborates as part of a team of physicians and nurse practitioners with regular meetings.
  • Conducts patient visits in the field for those requiring more intensive symptom management as requested by hospice team.
  • Reviews medications and recommends changes appropriate to individual cases.
  • Serves as PCP for patients when applicable.
  • Travels throughout the Central Oregon service area, including La Pine, Crescent, Christmas Valley, Prineville, Madras, and Sisters.
  • Takes calls M‑F from 8:00 AM to 5:00 PM.
  • Communicates findings to the Medical Director and the rest of the IDT involved in the case.
  • Adheres to the PIC mission and values.
Required

Skills & Qualifications
  • Nurse Practitioner is and shall remain duly licensed to practice medicine in the State of Oregon. Nationally Certified Family or Adult Nurse Practitioner preferred.
  • Requires excellent understanding of pharmacology, interactions, and use of controlled substances.
  • Professional Standards;
    Compliance with Law. Nurse practitioner shall at all times comply with the ethics and professional standards of the medical profession; applicable federal, state, and local laws, rules, and regulations;
    Partners In Care’s policies and procedures; and the rules and regulations that are applicable to Partners In Care, including accreditation and certification requirements.
  • Drug Enforcement Administration. Nurse practitioner has and shall at all times during the course of employment maintain registration with the Drug Enforcement Administration (“DEA”) and any applicable state agency without limitation on the Medical Director’s authority to prescribe drugs under such registration and shall provide current proof of such registration upon Partners In Care’s request.
