DEPARTMENTAL

SUMMARY:

The Quality Management (QM) department provides essential services to St. Charles Health System (SCHS) including: quality improvement expertise and support; data analysis; regulatory affairs, accreditation and licensing expertise and support; environment of care safety programming, expertise, and support; emergency preparedness; policy and document library management and support; data abstraction; integrity of clinical documentation; the patient experience and grievance program; and risk management and patient safety program.

Responsibilities
  • Facilitates quality improvement initiatives within assigned service lines and committees in alignment with the organization’s True North objectives.
  • Supports the Patient Safety program across assigned facilities and service areas, serving as a Subject Matter Expert for Root Cause Analysis, Safety Culture, National Patient Safety Goals and other evidence-based programs as needed.
  • Collaborates with stakeholders to assist with data analysis, goal selection, process improvement, and development of sustainment plans; develops, coordinates, and facilitates implementation of systems for risk identification, investigation, and mitigation of patient safety concerns.
  • Works with Quality and Safety teams to meet regulatory and accreditation requirements; partners with Lean Improvement Office to ensure a Lean approach.
  • Maintains focus in a fast-paced environment and adapts to changing priorities.
  • May review and provide feedback on the work of other caregivers (non-managerial).
  • Utilizes quality and safety improvement principles (e.g., PDSA, A3) to design, develop, implement, and sustain True North initiatives.
  • Collaborates on data collection, analysis, and development of auditing tools and dashboards.
  • Supports system safety and quality councils and committee meetings; participates in survey readiness and rounding activities.
  • Performs quality and patient safety reports, analyzes data, identifies trends, and makes improvement recommendations.
  • Reviews Safety Alert System reporting and responses; manages escalation, RCA, and apparent cause analysis.
  • Identifies sentinel events and leads investigations, RCAs, and FMEA to drive system and process improvements; assists with reporting to state and accrediting agencies.
  • Proactively conducts surveillance and evaluates patient safety risks across facilities and service areas.
  • Provides guidance on patient care issues with safety/risk or legal implications; collaborates on potential EMTALA and HIPAA issues with Compliance.
  • Supports patient complaint and grievance response letters; guides disclosure of unanticipated events to patients and families.
  • Collaborates with Subject Matter Experts to disseminate learnings via Safety Alerts and advisories.
  • Supports the organization’s vision, mission, and values; partners with Lean Office on process improvement initiatives.
  • Acts as a champion of change and helps create sustainment plans; maintains a safe environment for staff, patients, and guests.
  • Upholds professionalism and confidentiality; complies with laws, policies, and procedures; participates in investigations as needed.
  • Contributes to a healing environment and intentional relationships that enhance the care experience.
Qualifications

EDUCATION

  • Required
    :
    Bachelor of Science in Nursing, Registered Nurse.
  • Preferred
    :
    Master’s degree in nursing, public health, healthcare management, data analytics, or statistics.
