Case Manager

Evergreen Youth & Family Services is a leading non‑profit organization dedicated to empowering youth and families in Bemidji, MN, and the surrounding communities. We provide a range of comprehensive services aimed at supporting young people and their families to overcome challenges and thrive. We are seeking a dynamic and passionate Housing Case Manager to join our team.

• Provide individualized case management, including safety and needs assessments, counseling, advocacy, referrals, and coordination with community services
• Maintain accurate case documentation and reports, and manage data in HMIS and Evergreen databases
• Build and maintain strong professional relationships with clients, Evergreen team members, and community partners
• Assist youth with housing searches, applications, and navigating landlord‑tenant responsibilities
• Support youth through housing transitions and prevent evictions when possible
• Conduct skills‑building groups and participate in outreach, advocacy, and community education
• Share in reporting for funders, board updates, and agency needs
• Other duties as assigned to support Evergreen’s mission

• Applicants 21+ with experience or interest in working with youth experiencing or at risk of homelessness
• Team players who can work flexible hours, including evenings, weekends, and holidays
• Strong communication, organization, and advocacy skills
• A heart for helping youth thrive and navigate housing challenges

$20–$25/hour
