Join Sanford’s Inpatient Behavioral Health Hospital in Bemidji as a full-time RN Care Manager or Social Worker. This Monday through Friday, day shift role offers a consistent 40-hour workweek with no nights or weekends. You will collaborate with an interdisciplinary care team to support patient care coordination, discharge planning, and continuity of care in an inpatient behavioral health setting. • Provide care coordination, transitional care planning, facilitation and coordination of discharge needs with involvement of the patient/family and collaboration with licensed practitioners and inter-professional team members.
• Enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care.
• Promote patient wellness through evidence-based practice, improved care outcomes, efficient utilization of health services, and appropriate level of care for patients during hospitalization and after discharge.
• Provide discharge-planning education and make referrals to inter-professional teams when necessary; initiate links with post-acute external care resources.
• Identify patient educational needs and discharge planning issues, and collaborate to expedite early discharge planning.
• Advocate for the patient across care settings to achieve desired outcomes for clients and healthcare agencies.
• Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care.
• Collaborate with other inter-professional colleagues, including physicians, to plan, implement and evaluate care.
• Demonstrate competency within the full scope of nursing expertise and apply age/population-specific standards in the assigned clinical setting.
• Care for patients across all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up along the care continuum.
• Plan and coordinate care, patient assessment, and education for patients, caregivers, and family; triage and various nursing interventions.
• Operate within the Nurse Practice Act and Administrative Rules of the state of practice and licensure; follow the Code of Ethics for Nurses as published by the ANA.

• RN Care Manager
• Bachelor’s degree in Nursing strongly preferred. Associate Degree in Nursing (ADN) required if Bachelor’s degree is not obtained. Graduate of an accredited nursing program.
• Current, active, and unencumbered Registered Nurse (RN) license with the Minnesota State Board of Nursing.
• Wage Range: $34.00 - $51.00
• Social Worker
• Bachelor’s degree in Social Work from an accredited curriculum required.
• Must possess a license in good standing in state of practice:
  In Minnesota:
  Licensed Social Worker (LSW).
• Wage Range: $30.00 - $36.00
