Every Mind is hiring a Family Care Manager to join our Project Wellness team at Martin Luther King Middle School in Beltsville, MD. This role is responsible for direct care management services within a school and community-based program designed to integrate a system of resources and services for youth and families and to support them in achieving their goals.

- Completes comprehensive needs and strength assessments with youth and families resulting in individualized service plans and goals and appropriate crisis intervention or support as needed.
- Consults and collaborates effectively with colleagues, school staff, and other community agencies to provide comprehensive services for clients. Offers information and referral support for community members, school staff, etc.
- Provides ongoing social services to clients which may include securing needed resources, providing necessary referrals, and ensuring any court-ordered requirements are met. Assists with completing applications for programs and services that align with service plan goals.
- Maintains compliance with School Based Services contract including:
Meeting productivity requirements, maintaining minimum caseload requirements, and timely submission of data to support program reporting requirements. - Conducts psycho-educational groups, educational workshops, and resource presentations as appropriate.
- Incorporates evidence-based practices and individualizes activities to support students and families with a variety of physical, emotional, and social needs.
- Maintains accurate, thorough, and timely documentation of client records. Documents and evaluates services provided and client progress, updates service plan at regular intervals.
- Manages funds in assigned budget categories and utilizes assigned client needs funds to support service plan and client goal achievement.
- Assists with seasonal events, such as camp registration, school supply drive, holiday giving program, and other activities for the school community.
- Provides translation and interpretation services for clients and others as time allows.
- Utilizes regular supervision with Manager to evaluate and improve the quality of care management services and ensures that services are culturally appropriate for the client.
- Maintains client confidentiality according to Every Mind Confidentiality Policy.
- Other duties as assigned.

Qualifications Strong Personal Resilience and Mental HealthGiven the high-stress nature of the role, it is essential that candidates possess a high level of personal resilience and maintain stable mental health. This ensures that they are well-equipped to provide effective support and maintain the professional standards required for the position.
EducationBachelor’s degree in social service field required.
Experience- 1 year of successful experience in the human services field, required.
- Experience with case management service delivery preferred.
Strong interpersonal and communication skills. Demonstrated organizational and community building skills. Ability to manage multiple priorities. Accurate record keeping and data entry proficiency. Bilingual Spanish required.
AbilitiesEffectively interact and work collaboratively in a flexible work environment with a variety of constituents including CEO, board members, clients and family members or caretakers, interns, volunteers, staff, and community partners. Ability to express ideas with clarity, confidence, and sensitivity. Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.
