• Support patients with serious, life-limiting illnesses and their families.
• Coordinate care with a multidisciplinary team.
• Provide end-of-life symptom management and education.
• Develop individualized care plans.
• Coordinate with facilities and contractors as needed.
• Ensure documentation compliance.

• ADN required; BSN preferred.
• Current WI or IL Registered Nurse license, with ability to obtain the other (must apply within 30 days of hire).
• Hospice or Palliative Care experience is preferred.
