Peace Health RN Navigator – Remote
Peace Health seeks a RN Navigator for a full‑time, Day position. Salary range $48.52–$72.78.
Remote, service line clinic in Oregon & Washington. Must hold RN licensure in WA & OR; can obtain AK licensure as requested.
Essential Functions
- Patient and Family Liaison – single point of contact for patients and families, telephone triage, education, communication with surgical team, scheduling, follow‑up calls.
- Clinical Liaison – collaborate with service lines, notify surgeons of critical patients, coordinate multidisciplinary discharge, reconcile medications, prepare discharge instructions.
- Clinical Review – develop and maintain guidelines, policies, protocols, chart review, data collection for cardiac surgery outcomes.
- Educator – develop patient and clinician education programs, facilitate classes, preoperative teaching for inpatient add‑on cases.
- Patient Care Planning – ensure understandable treatment plans, coordinate scheduling of tests and specialty appointments, initiate referrals to community resources, follow patients through continuum of care.
- Patient/Family Needs Assessment – assess clinical, emotional, spiritual, psychosocial, financial needs.
- Multidisciplinary Planning – coordinate planning conferences and education, develop concise patient summaries, follow up with multidisciplinary heart failure team.
- Program Growth – guide appropriate service line program growth per governing bodies and best practice.
- Quality Improvement – facilitate QI initiatives, develop metrics.
- Other duties as assigned.
Qualifications
- Education:
Accredited School of Nursing degree. Preferred Bachelor of Science in Nursing or Master’s Degree. - Experience:
Minimum 5 years RN experience with 3 years caring for CV patients in acute or outpatient settings (Cath Lab, CVICU, EP, cardiac stepdown, ambulatory cardiology). Management and program development experience desirable. Self‑directed work ability. - Licensure: RN licensure in WA and OR required upon hire; ability to obtain AK licensure.
- Credentials:
Defibrillator Basic Life Support upon hire. - Skills:
Effective written and verbal communication, computer skills (MS Office), interdisciplinary team collaboration, data collection and interpretation.
Working Conditions
- Lifting – patient handling up to 35 lbs without assist. Fine motor skills. Push/Pull up to 45 lbs. CPR force 100–125 lbs.
- Environmental – exposure to biohazard, body fluids, airborne particles, noisy environment.
- Mental/Visual – normal vision, hearing; glasses/contacts allowed.
