Sapphire health services is currently seeking a dedicated individual to join our team and uphold our commitment to excellence.
working as a caregiver in our community based care facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with residents, their families, and care team to provide personalized care that meets each individual's unique needs and preferences.
responsibilities- provide assistance with activities of daily living (adls).
- monitor and report any changes in residents physical or mental health.
- assist with serving resident meals.
- provide companionship, emotional support, and meaningful engagement to residents.
- assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.
- a caregiver will maintain resident confidentiality.
- encourage clients to participate in activities that promote physical, mental, and emotional well-being.
- support clients in maintaining independence in their daily routines and decision-making.
- ensure a safe environment by identifying and addressing potential hazards in community setting.
- respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
- collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.
- communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.
- present for staff meetings and training.
- other duties as assigned.
- must be 18 years of age
- nac/cna or nar or hca acquired within 120 days of hire
- high school diploma or equivalent required.
- previous experience in caregiving, home care, or community-based care is preferred.
- ability to work independently and as part of a team.
- patience, empathy, and a genuine desire to help others.
- good organizational and time management skills.
- ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.
- must be able to stand, walk, and reach for extended periods.
- ability to pass a background check.
